Dowód osobisty (Polish ID card) + bank statement PDF from your Polish bank (mBank, PKO BP, ING, Santander, etc.).

Identity documents

Proof of address

All documents: issued within 3 months, full name and Polish address visible, PDF format.

For Ukrainians in Poland

Your proof of address must be a Polish document. A Ukrainian document will not work for a Polish address.

Use a Polish bank statement or utility bill showing your Polish address.

Name transliteration

Your passport and Polish documents may spell your name differently (e.g. SHEVCHENKO vs SZEWCZENKO). Make sure your account name matches your passport spelling.

Common issues in Poland

Name mismatch between documents. Different Latin spellings across passport and Polish bills cause rejections. Account name must match passport.

Screenshots rejected. Download the actual PDF statement from your bank app or online banking.

Ukrainian POA for a Polish address. Your proof of address must come from a Polish institution. Ukrainian documents do not confirm a Polish address.

Missing back side of ID. Both Dowód osobisty and Karta Pobytu require front and back. Do not skip the second photo.

Old documents. Only documents from the last 3 months are accepted.
