Ukraine: proof of address that actually gets approved
Upload the Витяг з реєстру територіальної громади from Дія as a PDF, or a bank statement in your own name that shows your address.
What passes
- Дія — «Витяг з реєстру територіальної громади» (registration certificate), saved as a PDF. This is the document that passes most often.
- A bank statement («Рух коштів») downloaded as a PDF. monobank and Universal Bank work best; Raiffeisen (MyRaif) and A-Bank also pass.
- Put it in your own name, with the address matching your sign-up form. Aim for something issued within 3 months, though the registry extract has no expiry.
What gets rejected
- A passport or ID card. That proves who you are, not where you live.
- A document in a relative’s name, or a receipt with no name on it.
- A «безстрокове» certificate with an old issue date. A distant issue date fails even when the document never expires.
- A photo or screenshot of an app screen instead of a downloaded PDF.
- Re-uploading the same file five or more times, which can trigger a hard block.
If you’ve relocated
Living in Poland, Germany, or Czechia is fine. Use a document that shows where you live now: a statement from your local bank (PKO, mBank, ING, Millennium, or Revolut/Wise), a rental agreement in your name, or a local utility bill.
A Ukrainian ВПО (displaced-person) certificate or an old прописка won’t work, since the address has to be your current one.
